What to Bring
- Drivers license or photo ID
- Insurance card (if you have insurance)
- Credit card, check, or cash for your co-pay or visit fee
- A list of your medications of the medications themselves
- Minors require the written consent of a legal guardian prior to evaluation
Sign-In
On arriving at Advanced Medical Care please sign in at the front desk. If you have not registered online, you will then be asked to review our Privacy Practices and to complete the following forms. You may print these out and bring in completed forms at the time of your appointment or if you wish to save time you may FAX them in, along with the front and back of your insurance card, to 661.294.0931.

- Scan your insurance card, driving license
- Collect your co-pay
What to Expect at your Visit
Following sign-in and completion of the required forms your visit will consist of:- A Medical Assistant will inquire into the reason for your visit and measure your vital signs.
- A Physician or Physician's Assistant will evaluate you.
- You will be provided with information noting the findings from your visit, the physician's treatment recommendations, and follow-up instructions. You will receive a copy of these instructions upon check out and payment of your visit.
